Eligibility: 62 and Older OR 55 and Older with a Disability:

- Hours: 10:00 AM to 4:00 PM EST
2. Required Documents:
- Photo ID for each household member
- For those aged 55 or older needing accessibility features: Provide your physician’s contact information (full name, telephone, and fax numbers) for verification.
3. Application Submission options & Deadline:
